If I remember correctly, I tended to do it this way:
Terra, Sabin, Shadow, Mog
Celes, Edgar, Locke, Setzer
Relm, Strago, Gogo (with Blitz, Magic and Tools), Cyan
I usually only had three or four characters with Ultima: Terra, Celes, Relm and sometimes Strago. Putting Gogo with a group where someone has magic and giving him/her magic as a command will place Ultima in the options.
If you look at the strongest section, you'll see how I equipped some of my characters. I did it this way to have a strong magic caster on each team, with a strong physical attacker and then some good all around characters. None of the characters are weak if equipped correctly and used in the way they are meant to. WIth Relm's group, you can take control of a behemeth and have it kill itself, for one thing. Her control command is often overlooked.
